I would like to use BV 5 as a tool for accepting applications for children to attend summer bible classes. I have seen where other applications offer product pages where you can add a custom field for the customer to enter information, not sure about BV5. In my case, I would like to allow the parent to include everything from the child's name, school grade, medical conditions, etc. Will BV5 do this out of the box or should I just create a form to capture the information?

Thank you for the reply. It appears that will work just fine - I have about 15 text fields that need to be added but otherwise what you have looks perfect for the application. Can you give me a pointer for where to look in the product admin that will get me started?
