How do I add dynamic questions to the ContactUs page

Hi
Looking at the vb code behind ContactUs.aspx there seems to be the functionality to load the page with a series of questions of various types at runtime. Problem is that I can't find anywhere in the Admin ares to set these questions up.
Can anybody help?

Thanks that helped me get the Contact Us form set up. One problem, when I got the email it had
Correspondence from [[Site.SiteName]]
in the body. Obviously not what was intended. Where is the config setting so that the Contact Us email sends out the Store Name?
