Estimate Shipping - Country/State/zip code fields - State missing

We have both a staging site and production that are using the same version of BVC5.7.1, however on the staging site, the 'Estimate Shipping' window has the country drop-down (USA only), then the State drop down and then a zip code field.
However on our production version, only Country and zip code fields show up. How do we enable the state drop-down?

I have resolved this issue with a workaround in the code. I just modified the EstimateShipping.aspx.vb file and changed
State.Visible = True 'Me.RegionIsRequired
in the page_load event to force State to display. Now all my FedEx shipping rates appear
