Bulk Order Form - Express Checkout Form?

I searched through the forums, and found information about the bulk order from storeyourmedia.com, for BVC2004. I emailed him to ask if he had a new version for BVC5 and he replied that BVC5 has a built-in Bulk Order Form.
I haven't seen it before, can someone direct me where I enable the bulk order form?

I am working on a form where if you know the sku's , you can enter them directly. This would be like if user has catalog in hand, knows what they want. the SKU text box would behave like Google ajax control, if you type first few letters it shows 10 sku that start with what you have typed so far. let me know if there is any interest by anyone else.
