When deleting a product, the custom URL is not deleted, so the URL cannot be used again without going to Content > Custom URLs and deleting it manually. |

Also, looks like you can have unlimited duplicate custom URLs. Perhaps a check for existing custom URLs when saving a product, or whatever, will eliminate issues brought about by cloning products, etc. |

I've run into another issue related to this one on a recent project.
A client may change the URL for a product a couple times, testing whether they prefer dashes or underscores in the name. Every time, a new custom URL is saved but the old one isn't deleted, so the product apparently ends up with errors. |
