I see the email templetes but how do I use them? For example in the contact us page it shows "please fill out the form below" but there is no form to fill out. I have a contact us email templete but how do I show it in my contact us page?

You can create your form by going to BVAdmin > People > Contact Us Config, which is a separate feature from the email templates. |

What is the best way to sort the fields on the customer service form?
The form creation module does not contain any sorting features..

We're looking at providing a sort option in Service Pack 2 that would allow this. |
